Home Based Care

At East Rand Palliative Care, we bring compassion and medical expertise directly to your home.

Our team of palliatively trained nurses specialise in providing personalised care that respects the dignity and needs of each patient. Our comprehensive approach ensures that every aspect of care is seamlessly integrated, including consultations with our medical doctor, social workers, and other professionals involved in the patient’s care.

What Our Home-Based Care Offers:

Personalised Visits

Our palliative care nurses visit patients’ homes regularly, tailoring care to individual needs. Visits are scheduled between 08:00 and 16:00 on weekdays to ensure consistency and responsiveness.

24/7 Support

While our nurses are not on-site at all times, they remain accessible by phone after hours for medical emergencies, offering peace of mind for patients and families.

Medical Coordination

Doctor visits are arranged as needed, based on nurse assessments and the doctor's discretion. Every patient’s treatment plan is reviewed regularly to ensure the best possible care.

Comprehensive Care

We provide pain and symptom management, nursing interventions such as wound care, nutritional advice, and emotional support for both patients and families through our dedicated social workers.

Additional Support

For patients needing continuous care, our nurses can arrange for a trained care worker to provide ongoing assistance, with this service available at an additional cost.

Caregivers

Specially trained caregivers are available to visit patients in their home to offer emotional support, companionship and basic hair care.

We are a community based organisation whose mission is to provide high quality holistic and non-discriminatory palliative care for persons with life-threatening illnesses and their families.

How To Access Palliative Care

Patients needing palliative care can contact us themselves, via a family member, patient’s doctor, social worker or other appropriate person. The important thing to note is that the patient must want palliative care and be aware that we have been approached. Patients can be registered by phoning the Intake Officer on 011 422 1531 between the hours of 9am and 3pm Monday to Friday or email intake@erpc.org.za  Download the Medical Report Form.

Once intake documentation has been completed and a medical report from the patient’s doctor has been received, a nursing sister from East Rand Palliative Care will do an assessment visit.

Paying For Palliative Care

Most medical aids have a palliative care benefit and all accounts for care and medication are submitted to them for payment. Non-medical aid patients are charged a set fee at the end of each month but provision of our services is not dependent on the ability to pay.
